Center for Medicare & Medicaid Design Solutions

The Center for Medicare & Medicaid Design Solutions is dedicated to transforming public programs by strengthening integration across Medicare and Medicaid, aligning financing and delivery systems, and supporting accountable, scalable innovation. This work will help states, health plans, providers, and partners move beyond fragmented programs toward coordinated systems that deliver better outcomes and more effective use of resources.

Focus Areas include:

• Medicare–Medicaid Integration: Advancing models that align financing and delivery to reduce fragmentation and improve patient-centered outcomes.
• Alternative Payment Models & Demonstrations: Informing and designing CMMI and state-led payment models that incentivize quality, innovation, and value.
• Behavioral Health, Primary Care & Social Needs Alignment: Strengthening coordination across medical, behavioral, and social supports to manage total cost of care and improve quality.
• Governance, Implementation & Performance Infrastructure: Building roadmaps, governance structures, and performance dashboards that make integrated care operational and sustainable.

Through this Center, we will work closely with our clients and partners to advance Medicare–Medicaid integration and inform alternative payment models and demonstrations. We also strengthen alignment across medical, behavioral, and social supports and build the governance and performance infrastructure needed to make integrated care operational and sustainable.

Center for Patient-Centered Transformation

The Center for Patient-Centered Transformation supports organizations bringing patient-centered innovations to market in ways that are scalable, sustainable, and aligned with evolving policy and payment landscapes. We work across the health innovation ecosystem, including technology companies, care delivery organizations, payers, employers, and investors, to help solutions succeed in real-world systems of care.

Focus Areas include:

• Market & Policy Readiness: Validating market fit for patient-centered models, including home-based, virtual, maternal/child, serious illness, and behavioral health solutions.
• Value-Based Payment & Reimbursement Strategy: Navigating Medicare Advantage, Medicaid managed care, employer-sponsored coverage, and alternative payment models that support adoption and scale.
• Commercialization & Go-to-Market Strategy: Aligning outcomes, equity, and ROI while positioning organizations for long-term success.
• Technology, Data & Interoperability: Operationalizing infrastructure for interoperability, analytics, and performance measurement to demonstrate real impact.
